Clinical Outreach Specialist - RN - PRN Weekends The Care Center is a team of Care Coordinators, and Clinical Outreach Specialists (LPNs & RNs)-PRN who support Pre and Post Assessment activities. The team is often the first and/or last touch with the member; establishing rapport and leaving a positive member experience.Using a member-centered approach, the Clinical Outreach Specialist engages with patients before and after they have completed a Health Assessment by a Matrix Network healthcare professional while in nursing homes, skilled nursing facilities (SNF), telehealth and in-home visits or working in an Employee Health and Wellness Service (EHWS) setting in a client location. This critical role conducts telephonic outreach to gather responses to pre-assessment questions and helps the member prepare for their upcoming visit. Post-assessment, the Clinical Outreach Specialist is also responsible for following up with members to provide lab results, address questions/concerns, and provide guidance on follow up critical for the next step in the member's care. Qualifications Educational Requirements: --- Current unrestricted licensure to practice as a Registered Nurse (RN) in the state(s) to which the candidate is assigned --- Maintains nursing license in good standing Knowledge, Skills and Abilities: --- Minimum three (3) years of nursing experience --- Home care experience preferred (not required) --- Telephone triage experience preferred --- Ability to provide telephonic support and education to members in a way they are able to understand This position is PRN and Weekend hours Matrix Medical Network is Proud to be an Equal Opportunity Employer
