Job Description Job Description We are seeking a compassionate,
highly organized Registered Nurse Clinical CareGuide to join our
Advanced Primary Care team in Scottsdale, AZ. In
thispatient-centered role, you will provide end-to-end support for
specialist anddiagnostic testing navigation, ensuring patients
receive seamless, coordinatedcare throughout their health journey.
As a key clinical partner, you willcollaborate closely with
providers, care coordinators, and support staff tohelp patients
achieve their best possible health outcomes. Compensation &

model. Key Responsibilities: Serve as the main point ofcontact for
specialist and diagnostic testing navigation, including
schedulingand follow-up. Support patients throughindividualized
care plans and promote proactive health management. Conduct
clinical assessmentsand provide guidance aligned with the Advanced
Primary Care model. Assist with caretransitions, including
post-discharge follow-up and coordination with communityresources.
Maintain accurate electronicmedical documentation. Provide
excellent customerservice and build strong patient relationships.
Perform other duties asassigned. Work flexible hours,including
mornings, evenings, or weekends. Travel between care centersas
needed; position is 100% on-site. Requirements: Current Arizona
RegisteredNurse license. BLS certification required. Experience
with ElectronicMedical Records (EMR), including Medicare-based
systems. Strong customer serviceorientation with experience in
patient-facing roles. Excellent verbal, written,and electronic
communication skills. Active AZ FingerprintClearance Card
required.
