Experience: Minimum 1 year of skilled home health experience
required EMR: Kinnser (WellSky) experience preferred What We Offer:

patients homes, including assessments, medication administration,
and care coordination Develop and update individualized care plans
in collaboration with the care team Educate patients and caregivers
on disease management and home care techniques Accurately document
all care using our EMR (Kinnser/WellSky) Communicate effectively
with physicians, team members, and families to ensure quality
outcomes Requirements: Active RN license in the state of Arizona
Minimum 1 year of recent skilled home health experience using OASIS
Strong clinical and organizational skills Ability to work
independently and manage a patient caseload effectively EMR
experience a plus (Kinnser/WellSky preferred) If youre looking to
