Job Description Job Description WEST VALLEY We are seeking an RN Case Manager Hospice to join our team! You will be responsible for the assessment, management of symptoms in the terminal diagnosis, and treatment of assigned patients as well as supervisory visits for LPN, HHA staff. Responsibilities: Administer nursing care to terminally ill, injured, or disabled patients Establish patient treatment plans with the IDG Monitor and report changes in patient symptoms or behavior Communicate with collaborating physicians or specialists regarding patient care Educate patients about health maintenance and disease prevention Facilitate referrals to other healthcare professionals and medical facilities Maintain accurate patient medical records Provide advice and emotional support to patients and their family members Qualifications: Previous experience in nursing in hospice Familiarity with medical software and equipment Ability to build rapport with patients Strong problem solving and critical thinking skills Ability to thrive in a fast-paced environment Company Description The office is located in Glendale. The case load is located in the west valley. You will drive throughout the west valley to visit patients in their homes to provide care. This is not an office position, however you would come to the office at least one time every other week for meetings. Company Description The office is located in Glendale.
